This assignment involves the case of Mrs. Velma Douglas, a 75 year old female patient admitted to the hospital for repair of a broken hip. She is 5’2″, 140 lbs with no known allergies. She has returned to the unit from the operating room with the following medication orders:

  1. Heparin 5,000 units subcut BID
  2. Furosemide 0.5 mg/kg prn for weight gain of 2 lbs (0.9 kg) or greater overnight
  3. 2000 mL of 0.9% normal saline solution IV to infuse over 24 hours, then decrease to 50 mL/hr
  4. Morphine sulfate 2 mg IV q1hr for moderate pain ( 4- 6 on pain scale); may give 4 mg q1hr IV for severe pain (7-10 on pain scale)
  5. Penicillin G 5 million units IV piggyback x1 now, then 2.5 million units q6 hr for a total of 48 hours
  6. Lispro insulin before meals and at bedtime based on the following sliding scale:
  • Blood glucose to 150: No coverage
  • Blood glucose 151-200: 2 units
  • Blood glucose 201-250: 4 units
  • Blood glucose 251-300: 6 units
  • Blood glucose greater than 300: Call provider
