I did practicum at Infinity Independent Living Services as a case Manager for low income people

-Objectives: Learning skill in establishing and maintaining relationships with site staff and clients, including personal of different culture, ethnic background and providing a back ground experience.

-Diagnose and resolve complex issues (financial assistant, housing and medical care) with clients from Hennepin County in Minnesota.

-I met client who are real serious struggling with life and came for help to get assisted with government resource.

-I did attend conferences and public health training.

Instructions for MPH 599 Final Practicum Paper (7095)

Final Paper Requirements: A double-spaced typed written report concerning your practicum position is to be prepared as a major part of fulfilling the grade requirements. The minimum acceptable length, excluding the title page and appendices is 12pages. The title page should include your name, the name and address of your employer, and the name of your director/supervisor(s). This paper may be written in the first person (using "I") since it is a discussion of a personal experience. Grading is based on the student meeting each of the elements below.

The following topics should be addressed in the final paper; however, feel free to include any information you feel would be of interest and beneficial for the betterment of the MPH education at Nebraska.

Abstract: Summary or overview of the key points of the practicum. (no more than ½ a page)

Introduction: Background of project including definition of the public health issue being addressed, its significance, and how it fits with the purpose/mission of the organization. Describe the organization overseeing the project(s), its purpose and/or mission statement, and funding sources. (approx. 2 pages)

Discussion: Describe your role in the project, including activities you were involved in. Assess whether the proposal goals and objectives were appropriate in addressing the problem. Include discussion of barriers that were identified and how you addressed them. (approx. 5 pages)

Personal Assessment: Describe how this practicum has contributed to an understanding of public health practice. Identify which of the public health competencies were applied during the MPH practicum. (approx. 2 pages)

Conclusions and Recommendations: Anticipated or suggested further action on the project resulting from project work and outcomes (approx. 2 pages)

Appendices: All materials developed by the student as a part of the practicum should be included in the appendices.

Evaluations: The supervisor evaluation needs to be included in the appendices.
